With local treatment as the mainstay, antibiotics are generally not required.

1. Acute treatment Control the infection and keep it dry.

(1) Mild treatment: removal of local scab, cleaning with 3% hydrogen peroxide solution and 75% ethanol at any time; adults can use hot salt water for wet dressing; keep the umbilical area dry.

(2) Abscess treatment: Abscess can be applied outside the umbilical area with golden ointment or physical therapy to limit the infection and promote abscess formation and rupture. Drainage should be made after the formation of an abscess.

(3) Systemic Infection Treatment: Patients with more pus or concurrent peritonitis and septicemia should receive a sufficient number of broad-spectrum antibiotics such as penicillin, and use effective antibiotics based on bacteriological findings.
(4) Supportive therapy: In case of systemic infection, care should be taken to supplement water and electrolytes. Fresh blood, plasma, or albumin may be properly administered to improve immunity.

2. Chronic phase treatment Small granulation wounds can be burnt with 10% silver nitrate and then coated with antibiotic ointment. Large granulation wounds may be surgically removed or electrocoagulated to remove granulation tissue. Keep the umbilical fossa clean and dry to heal.

Talcum powder should not be used in wounds with secretions to avoid irritating granulation.
